The goal of plastic surgery is to create an aesthetically pleasing and proportionate facial contour. This can be done by retracting a prominent chin or lengthening the chin in patients with a recessed chin. A patient's profile can be significantly altered by chin advancement or retraction procedures. These procedures have a significant effect on achieving overall facial symmetry. Approximately one in five patients who undergo rhinoplasty or facelift have a projection disorder of the chin. However, the patient is often unaware of this deficiency and is not informed that surgery can correct this disorder.

There are several surgical options for treating chin deformities. Alloplastic chin implants and bone augmentation are the two currently accepted methods of chin lengthening. Chin reduction techniques include bone shaping, called genioplasty, and direct chin reduction.

First of all, during the examination, facial analysis is used to determine whether there is an underlying occlusal (i.e. jaw closure and teeth position) and skeletal deformity or just protrusion or recession at the tip of the jaw. Before the operation, occlusal assessment should be performed together with standard facial photographs. If there is an occlusal problem, i.e. teeth closure, this is evaluated together with a dentist consultation. However, when only a cosmetic correction is desired, implant placement or bone advancement is performed.

It is important to consider the subliplar fold in planning chin implant placement. A large implant can increase depth and cause profile harmony imbalance. To prevent this, implant selection should be made correctly or bone advancement or recession surgery options should be considered.

Implant augmentation is generally not technically difficult and has a low complication rate. Furthermore, these implants can be easily placed, even with local anesthesia. This is a well-accepted technique for correcting mild to moderate jaw deformities.

Complication rates are similar to those of bone advancement or recession surgeries (sliding genioplasty). However, the postoperative recovery period is longer. In addition, this technique is more successful in correcting abnormalities in the 3 dimensions of asymmetry. Correction with implants is excellent for small to moderate anomalies. However, correction with bone genioplasty allows for the treatment of more complex deformities.
