Hair is one of the most variable characteristics of people. The color, length, density, structure and shape of hair characterize the differences of different races and ethnicities. Hair style and adornment change with age. Today, men as well as women attach great importance to hair fashion and products. When these are considered, it is understandable how hair loss causes anxiety and why there is so much interest in hair restoration. With the increasing interest in looking young in the population where the effects of aging occur, the interest in hair restoration increases at this rate. In addition, the protective effects of hair against cold, heat and ultraviolet rays are other factors that increase the interest in hair restoration.<\/p><div><p>It is essential for surgeons involved in hair restoration to know the basic anatomy and aesthetics of hair to achieve successful results. The character and structure of hair vary greatly depending on the developmental stage and location. Lanugo hair is soft, fine, light and colorless hair that is shed from the fetus in the womb around the 8th month of pregnancy. Vellus hair is the fine, light and almost invisible hair seen on the forehead and head that is present in the pre-pubertal period. Terminal hair is the thick, long hair that can be seen in various colors in adults. The subtypes of terminal hair are seen on the scalp, eyebrows, upper lip, chin, armpits, genital area and anterior chest wall. During puberty, vellus hair on the face turns into terminal hair and forms the beard. In male pattern baldness, which is seen in both sexes, terminal hair turns into vellus hair.<\/p><p>Male pattern baldness is characterized by the gradual transformation of hair from the terminal type (healthy, thick) to the vellus type (light, microscopic). This genetic condition is dependent on circulating androgens. Thinning hair first appears during puberty as the amount of circulating androgens increases. Initially, the hairline recedes and gradually progresses to leave only hair on the temporal (temples) and back of the head.<\/p><p>The main androgen found in male plasma is testosterone secreted from the testicles, while in women it is dehydroepiandrosterone sulfate, androstenodiol sulfate and 4-androstenodione secreted from the adrenal gland. Testosterone and proandrogens secreted in women are converted to biologically active dihydrotestosterone by the 5 alpha-reductase enzyme found in target tissues, causing androgenic hair loss in women and men.<\/p><p>As can be understood from the hair anatomy and physiology described above, optimal graft survey and ensuring the highest level of hair growth depends on transplanting more than just the hair strand alone.<\/p><p>Taking a bare hair as a micrograft deprives the hair of vital structures that will allow it to live. Slightly full and intact follicular unit grafts are much more successful. From this perspective, hair transplantation is not actually a hair transplant but a transplant of the hair strand or follicular units containing the hair strands seen below.<\/p><p>Follicular unit<\/p><p>The basis of hair replacement surgery is the transplantation of hair that is highly resistant to shedding, located in the form of follicular units at the back of the scalp, to the bald area. The concept of donor area dominance basically means that hair that is resistant to shedding is also resistant to shedding after being transplanted to the bald area.<\/p><p><strong>CLASSIFICATION OF MALE PATTERN BALDLE<\/strong><\/p><p>Male pattern baldness is classified into 8 types. In type I, there is minimal or no receding in the front hairline. In type VII, there is only a sparse layer of thin hair in a small horseshoe-shaped area in this area. Androgenic alopecia seen in women is classified into 3 types according to the amount of loss. Thinning of hair is usually much more widespread and in most cases the front hairline is preserved. The density and quality of hair is good only in the back of the head area. For androgenic alopecia seen in women, excessively high levels of circulating androgens or hormonal imbalance are not a requirement. In fact, androgen levels are normal in most. Androgen receptors in hair follicles are extremely sensitive to dihydrotestosterone, and this may or may not be due to genetic predisposition.<\/p><p><strong>OBTAINING FOLLICULAR UNITS<\/strong><\/p><p>Follicular units can be obtained from the back of the scalp in two ways; \u201chair-containing strip excision\u201d and \u201cdirect collection of follicular units from the donor area\u201d using perforators (punch).<\/p><p>Strip excision containing hair is called follicular unit transplantation (FUT). Since previously follicular units could only be obtained by separating the scalp into strips containing hair, direct strip excision came to mind when \"FUT\" was mentioned. This method became popular in the 1990s.<\/p><p>The method of directly extracting follicular units from the donor area with a thin drill, thus eliminating the need to cut and remove the hair in a strip, was first proposed by Dr. Wood as the \u201cWood\u2019s technique\u201d. This method was later called \u201cfollicular unit extraction\u201d (FUE). In fact, \u201cFUE\u201d is also a follicular unit transplant. In other words, comparing \u201cFUT\u201d and \u201cFUE\u201d in hair transplantation is completely wrong terminologically. The comparison should be made between \u201cstrip excision\u201d and \u201cFUE\u201d. The retention percentage of grafts obtained with the FUE technique is the same as those obtained with the FUT technique.<\/p><p><strong>STRIP EXCISION METHOD (FUT TECHNIQUE)<\/strong><\/p><p>Single strip excision was used quite frequently in the past because it was easy to apply and was frequently recommended to candidates for hair transplantation. However, it has many disadvantages. In this method, a strip is obtained by cutting an area between the ears on the scalp with a scalpel. The resulting surgical wound is repaired with stitches. In order for the scar to be invisible, the patient must always wear their hair long. An average of 1500 follicular units are prepared from this strip. In order to increase this number even more, the strip should be made either thicker or longer by extending it towards the ears. If it is made thicker, the tension will increase and the scars that will form after the stitches may widen. If it is made longer, this will cause the scar to be visible from the sides. Another method used to obtain more hair from a strip of the same length and width is to obtain micrografts by separating the follicular units that come out into individual hairs under a microscope in order to increase the number. However, it should not be forgotten that dividing the follicular unit grafts as a whole results in a slower growth rate. If the hair follicle is damaged while the donor strip is excised or the grafts are prepared, the viability of the hair decreases by 50%.<\/p><p><strong>FUE TECHNIQUE<\/strong><\/p><p>FUE is a hair transplantation technique where the extraction of follicular units is different but the implantation is the same as FUT. With this method, follicles grafts are taken from the back of the head with special micro-drills without using stitches and transplanted.<\/p><p>The most commonly used donor areas are the back of the head and the back of the temples. The FUE method allows patients with insufficient hair in these areas to take grafts from other parts of their bodies.<\/p><p>On the day of the operation, the donor area where the hair will be taken is cut to a length of 1-2 mm. The patient lies face down on the operation table. Local anesthesia is applied to the entire donor area. Grafts are taken from the donor area with the help of 0.6, 0.8 or 1 mm micro drills. The taken grafts are quickly separated according to the amount of hair they contain and their thickness. It is important for the viability of the grafts to be as short as possible between the taking and planting of the follicles. Afterwards, the patient is turned to the supine position and the area where the hair will be planted is anesthetized with local anesthesia and the original hair directions are determined and the channels where the follicles will be planted are opened with a mini scalpel in accordance with these directions. Here, the front hairline should be created in a natural way and should be indented and protruding, and single follicles should be used much more frequently to avoid an undesirable artificial appearance. It is also important for patients to know the direction in which they comb their hair. Although the wishes of our patients are decisive in where the follicles will be planted, we make personalized recommendations on this subject. An important detail here is that the surgeon's recommendations should be taken into consideration in order to avoid the shapeless appearance that may occur if the person who has had a hair transplant has hair loss in their original places in the area where the transplant will be made, i.e. if hair loss continues.<\/p><p>So how many follicular units are transplanted in a hair transplant session? There are many figures on this subject, from 1500 to 5000 follicular units transplanted in the written press and on the internet. Frankly, figures above 3000-3500 grafts seem misleading to me for advertising purposes. What is important here are factors such as the width of the area where the follicles will be taken, how angled the hair grows, and the tendency to bleed, and these vary from patient to patient. In addition, a minimum of 8 hours is required to take and transplant 3000-3500 grafts, and the longer this period, the more viable the follicles are, that is, their chance of survival. Therefore, when performing hair transplantation, we try to take the maximum grafts from the widest area we can and complete the transplantation process quickly. In addition to the factors we can determine before the procedure, factors that emerge during the procedure also affect the number of grafts taken. When considered from this perspective, taking an average of 3000 grafts and completing the transplantation process in the shortest time is a successful hair transplantation procedure.<\/p><p>At least 15 days before hair transplantation, drugs that increase bleeding (such as apricin, plavix) and herbal medicines should be stopped. It is recommended to stop alcohol and smoking in the days before the procedure. Cosmetic products applied to the scalp should also be stopped at least 1 week before. If there are scalp diseases, they should be treated before the operation. Those with hypertension and diabetes should use these medications without stopping. Patients with other chronic diseases should definitely inform their doctors about this during the examination before the procedure.<\/p><p><strong>AFTER HAIR TRANSPLANTATION PERIOD<\/strong><\/p><p>Traditional dressing is done by creating a layer on the surface with antibiotic pomade, covering it with gauze, wrapping it like a turban and covering it with an elastic protective cover. This dressing is removed by us after 2 days, the first hair wash is done and the washing process to be done once a day for the next 10-15 days is shown to our patients in practice. It is important for patients to use the prescription written during this period.<\/p><p>Pain\/discomfort that may last for a few days or weeks as part of hair transplantation, swelling that starts two days after surgery and can last for 2-4 days, redness and crusting that can last for 7-14 days, and telogen effluvium, where natural hair starts to grow again after about 8-12 weeks, are very undesirable but expected situations.<\/p><p>The transplanted hair enters a telogen (resting) phase for 2 to 4 months and then begins to grow again. Significant regrowth occurs in the 4th month, but it takes about 1 year for the final result to appear. Anxious patients may perceive the situation as very bad for about 4 months. Women have a harder time getting through this period. In addition to various cosmetic concealers and styling tricks, these patients may need to use a wig for a while.<\/p><p><strong>Postoperatif Komplikasyonlar<\/strong><\/p><p>En yayg\u0131n komplikasyonlar transplante edilen dokunun al\u0131c\u0131 b\u00f6lgenin y\u00fczeyinin alt\u0131nda s\u0131k\u0131\u015fmas\u0131yla ortaya \u00e7\u0131kan inflamatuar ve noninflamatuar lezyonlard\u0131r(p\u00fcst\u00fcller, pap\u00fcller, nod\u00fcller ve kistler). Ameliyattan 8-12 hafta sonra normal b\u00fcy\u00fcme zaman\u0131 ile ayn\u0131 zamanda ortaya \u00e7\u0131karlar. Frontal b\u00f6lgede \u00f6n sa\u00e7 \u00e7izgisi olu\u015fturulurken b\u00fcy\u00fck greftlerin kullan\u0131lmas\u0131na ba\u011fl\u0131 hiperfibrotik frontal kabar\u0131kl\u0131k da ortaya \u00e7\u0131kabilir.<\/p><p><strong>Postoperative Complications<\/strong><\/p><p>Patients are called two days after surgery to have their dressings changed and to have smaller, lighter bandages or none at all. Typical checks are at 16 weeks to check for early regrowth and at 6-12 months.<\/p><p>Many male patients should use 1 mg finasteride to prevent or slow hair loss and thus delay the multiple surgeries required for patchy baldness. Side effects such as decreased libido occur in 2 to 4% of patients and regress when the drug is discontinued. Topical minoxidil is useful in delaying hair loss and can be used in combination with finasteride or alone. Even if the patient does not use these drugs long-term, it is necessary to use them for 6 months postoperatively to ensure early regrowth of transplanted hair and to prevent shock loss. Finasteride should not be given to women planning pregnancy, but topical minoxidil can be used.<\/p><p><strong>HAIR TRANSPLANTATION IN PATIENTS WITH CURLY HAIR<\/strong><\/p><p>Patients with extremely curly hair often have low follicular unit densities. The narrow angulation of the follicular root and bulb makes microdissection more difficult and avoiding graft separation more complicated. Leaving more nonfollicular tissue around the follicular unit often solves this problem. However, this also means that a larger recipient area is required. The majority of female patients with this hair texture have more fibrotic recipient sites due to traction alopecia from long-term use of chemical or mechanical hair control devices. Excisional devices are often required for recipient site preparation in these scalps.
